GENERAL DESCRIPTION: Assist the Program Manager - Shared Living (SL) to coordinate and oversee assigned contracts and services within Goodwills Shared Living program.

) Assist the Program Manager with discussions, reviews, and training regarding Shared Living documentation and record retention in Therap and in individual, physical case files.
Respond to review findings from Program Facilitator - Shared Living (SL).
Complete assigned assessments (assisting the Program Facilitators as needed) to identify individual strengths, needs, safety risks, preferences, and abilities.
Work alongside Habilitation Specialists to receive training and verify competency over all habilitation programs and safety plans at implementation and as changes occur.
Train Shared Living Providers and Back-Up Providers to carry out all habilitation programs, safety plans, and corresponding documentation, as instructed, at implementation and as changes occur.
Deem Providers competent to carry out the programming and safety plans; obtain documentation of all training and competency verification.
Assist Habilitation Specialists as needed with identifying potential affects at progress in programs during monthly reviews.
Prepare for and actively participate in Individual Support Plan (ISP) development as needed in replacement of or to assist the Program Manager.
Assist the Program Manager with reviewing and approving Medium/Low General Event Reports involving Shared Living.
Assist the Program Manager with obtaining all Shared Living Provider (and Back-Up Provider) records and provide them to the Program Facilitator for retention.
Assist the Program Manager to ensure that Shared Living Provider setting requirements are met prior to and during placement according to regulations and the Final Settings Rule.
Assist the Program Manager with ensuring that Shared Living Provider (and Back-Up Provider) background, training, inspection, and Therap requirements are met prior to and during placement.
Assist with ensuring that Shared Living Provider reporting and enrollment requirements are met prior to placement.
Assist the Program Manager complete and document monthly home visits.
Work alongside Program Managers to fulfill Human Legal Rights (HLR) Review Committee responsibilities.
Assist with completing and seeking Interim approvals as needed, prepare and upload documents for HLR Meetings, retain copies of records presented during the HLR Meetings, communicate status with ISP Teams, and monitor status of referrals as well as supporting documentation on an ongoing basis.
Assist the Program Managers with Advisory Committee responsibilities.
Assist the Program Manager with habilitation leadership through active involvement in service provision.
Assist the Program Manager with phone calls, crises, emergency situations, behavioral needs.
Complete on-call duties as assigned.
Meet attendance and appearance requirements per Goodwill policies.
Perform work activities in conformance to Goodwill Values Statement which includes the values of Respect, Integrity, Compassion, Stewardship, Learning, Teamwork and Entrepreneurial Spirit.
Immediately report any wrongdoing, unethical or improper conduct, fraud, fiscal mismanagement or misappropriation of funds, etc.

) Encourage and do not hinder in any way an employee making a report pertaining to Goodwills Corporate Compliance Program Policy and Procedure or making a report of work injury.

EDUCATION, TRAINING AND EXPERIENCE: Bachelors degree in related field with a minimum of three years of experience in the field of developmental disability service delivery and at least one year leadership or management experience, or any equivalent combination of experience and training that provides the required knowledge, skills and abilities.
Must have a valid drivers license issued in the State of Nebraska and meet insurance requirements.
PHYSICAL REQUIREMENTS OF THE ESSENTIAL JOB FUNCTIONS: Ability to communicate in spoken conversation, adequate volume speech, and have normal hearing for conversation, telephone use, and emergency information systems.
Ability to be independently mobile on all surfaces and situations; to move safely about the work area.
Visual acuity, depth perception and field of vision within normal range.
Physical stamina to work at least 8 hours per day and be able to travel to conferences, meetings, job seekers homes, etc.
Physical ability to safely lift up to 35 pounds.
